I believe Kirk at Pacenti had a go at an IS integrated set up years ago, but honestly, with the variations in fork leg diameter, axle spacing, and IS/POST/Flat mount specs, it is far easier to use them as separate pieces.
What is the hurdle you are experiencing; Design, creating the parts, or fabrication/fixturing using the individual pieces?

I want to experiment with making a plywood fork and it would be nice there was a cots part I could use as both the dropout and disc brake mount. Also, if it was long enough to incorporate the disc brake mount it would have more area to interface with my wood laminate. I could imagine also using one for the non-brake side and just cutting off the brake mount- to provide a flange with a large interface area.
